What is the retention rate of fully-functional root-filled premolars at five or ten years when the crown is restored with:

1. A direct composite with no cuspal coverage;
2. A direct composite with cuspal coverage;
3. A laboratory-made gold or ceramic onlay;
4. A full gold, porcelain fused to metal or all-ceramic crown?
